Please take this home and give it to your parent or guardian. Return the completed form to the office . If you are a freshman and need the required proof of dental exam, the SBL Dental Services will meet that requirement.

Athletic Passes are available for sale in the office! The passes are valid for all HOME high school and junior high athletic games and tournaments in the Arthur CUSD #305 district except IHSA and IESA post-season tournaments. Passes are $25 for students, $50 for adults and $25 for senior citizens. Passes may be purchased in the office at Arthur Lovington Atwood Hammond High School, Arthur Grade School, Lovington Grade School or Atwood Hammond Grade School.
